Only in Karnataka we have Rule 14 (Ammended & Inserted) There is no specified Format is Altogether A different issue . There is an appeal rule in Para 6&7,8 of the Rules And Since you Wish to apply to BBMP , There is an RTI Cell to accept your Application For all thier 469 Pios in one place and give acknowledgement , The First appeallate authority in BBMP inspite of All our efforts Do not Carry out anything for 45 days , and all Appeals end up at the KIC . KIC Accepts Complaints under Section 18 and you have a gujarath high court order where in you can File Both first appeal & second appeal with different reliefs .
