1) A CPIO can reject a RTI request where the payee's name
in IPO is blank

2) Your query is hypothetical in nature. No stricture lies
against a CPIO for item #1.

3) Likewise, the CPIO can return / reject request when the
fee differs from the prescribed amount.

The case Law of Suprme Court, is that procedural; matters
/ technicalities should not come in the way of delivering
"justice". "Procedure is a mere handmaiden to justice"
etc etc.
